PURPOSE OF AWARD
To recognize alumni of Phi Lambda Sigma who have exemplified leadership qualities in support of the profession of pharmacy.
NUMBER OF AWARDS & RECOGNITION
One alumnus may be chosen annually, per chapter. The individual chosen by the chapter for this award will be listed in the annual awards booklet at the Phi Lambda Sigma awards banquet. In addition, the chapter will receive a certificate to present to the awardee.
One alumnus will be selected from the chapter submissions as the National Alumnus of the Year. The recipient of this distinguished recognition will receive an engraved plaque, which will be presented during the Phi Lambda Sigma awards banquet in conjunction with the APhA annual meeting. Additionally their name will appear on the perpetual plaque maintained at the Phi Lambda Sigma national office. The award recipient will be given the opportunity to make a brief address during the awards banquet.
The individual chosen must have a proven record of accomplishment of servant leadership in any one or more of the following areas; professional practice, education, association work, community service, or mentoring. The individual must be a member of Phi Lambda Sigma. Please note that local alumni winners do not need to be alumni of the local chapter of Phi Lambda Sigma. The nominator is responsible for completing the nomination form and submitting the award nomination packet. The nomination packet is to include the following items:
- Nomination form
- Current Curriculum Vitae or Resume of the nominee
- Optional: up to 2 letters of recommendation from individuals affiliated with a collegiate chapter such as Chapter President, Chapter Advisor, School/College of Pharmacy Dean, etc.
