Chapter Member of the Year Award

The “Chapter Member of the Year” award is available to one member from each chapter on an annual basis.  This individual is chosen by their respective chapter based on contributions to pharmacy leadership.  Although no formal nomination materials are required to be submitted to the national office, please fill in and return the form below in order that your chapter recipient is eligible to receive an official certificate.

Your chapter may present the certificate at any time throughout the year, however if the completed form is received by February 15th, the name of your recipient will be listed in the program of Phi Lambda Sigma events at the annual House of Delegates meeting.

PHI LAMBDA SIGMA-GLAXO SMITH KLINE-AFPE FIRST YEAR GRADUATE SCHOLARSHIP

PURPOSE OF AWARD

To encourage outstanding Phi Lambda Sigma members to pursue the Ph.D. in a college of pharmacy graduate program.

 

NUMBER OF AWARDS AND STIPENDS

One (1) Phi Lambda Sigma-Glaxo SMITH KLINE-AFPE First Year Graduate Scholarship at $7,500. 

The $7,500 may be used for any purpose decided by the awardee and faculty sponsor that will enable the student to have a successful program -- e.g., student stipend, laboratory supplies, books, materials, travel, etc. related to the program of graduate study. None of the funds shall be used for indirect costs by the institution.

 ELIGIBILITY

Applicant must be in the final year of a Pharm.D. program, and be a member of Phi Lambda Sigma. U.S. citizenship or permanent resident status is required.

APPLICATION PROCEDURE

  1. Letters of recommendation from two college faculty members who are acquainted with the student and his/her potential for graduate study.

  2. Name of the graduate school that the student plans to attend (if known).

  3. A one or two page statement by applicant elaborating reasons for wishing to attend graduate school.

  4. A list of special honors, awards and accomplishments in high school and college reflecting achievement and ability to succeed in graduate school. This information can be a vital part of the application.

  5. An official transcript of all collegiate grades and copies of GRE, SAT, and other national achievement test scores.

 

APPLICATION DEADLINE AND SELECTION PROCEDURE

Applications for the scholarship are due by February 15th annually. The Phi Lambda Sigma awardee will be chosen by a three-member Selection Committee composed of the Executive Director and President of the Society and either the President or Chairman of the Board of Grants of AFPE.

The recipient will be invited to attend an award ceremony during the Annual Meeting of the sponsoring Society held during the Annual Meeting of the American Pharmaceutical Association (APhA) in March. Funding for successful applicants will be provided in two equal stipends and upon receipt of confirmation that the student is enrolled in a pharmacy college graduate program for the Ph.D. Enrollment in a pharmacy college graduate program for the Ph.D. must occur not later than 16 months after completion of his/her B.S. or Pharm.D. program.

 

 

Phi Lambda Sigma-Procter & Gamble
National Leadership Award

Applicants are sought annually for the Phi Lambda Sigma National Leader ship Award. This award sponsored by Procter and Gamble is given annually to an individual who has exemplified leadership qualities in support of the pharmacy profession.

The individual chosen must have a proven track record of servant leadership in any one or more of the following areas; education, association work, community service, professional practice or mentoring. The individual need not be a pharmacist or a current member of Phi Lambda Sigma. Honorary membership will be bestowed upon non-member recipients.

The recipient would receive an engraved plaque which is presented during the Phi Lambda Sigma awards event in conjunction with the APhA annual meeting. Additionally their name will appear on the perpetual plaque maintained at the Phi Lambda Sigma national office. The award recipient will be given the opportunity to make a brief address during the awards event.

Deadline:  January 31st annually.

Phi Lambda Sigma New Chapter Grant

Phi Lambda Sigma National Pharmacy Leadership Society is pleased to announce the New Chapter Incentive Grants for leadership.  It is our mission “to foster and recognize leaders and to support leadership commitment.”  In keeping with this mission, it is our desire to reward leadership in a newly established PLS chapter through provision of this grant.  The nominee chapter should be one that has been chartered within the past year.

Criteria:

Phi Lambda Sigma chapter in good standing

Only one nomination per chapter

Project must involve at least ten (10) percent of PLS membership

bullet

Project must benefit at least twenty (20) people

A chapter may only receive this grant once

Chapters are eligible for this grant in their first chartered year

Projects must be completed no later than six months after the chapter has been awarded the grant 

 Deadline:   Applications will be accepted and funds will be distributed throughout the year.  All grant recipients will be recognized at the House of Delegates at the PLS/APhA Annual Meeting.

DESCRIPTION:

The Phi Lambda Sigma Founder's Award was established in 2003 to recognize a collegiate Phi Lambda Sigma member who has demonstrated exemplary leadership skills and service to the profession of pharmacy and/or the community, and is a role model and mentor to others.


CRITERIA:
The nominee must: 1) be a member of Phi Lambda Sigma in good standing; 2) have a minimum grade point average of 2.5 on a 4.0 scale; 3) have demonstrated exemplary leadership skills and service to the profession of pharmacy and/or the community; 4) be a role model and mentor to others; and 5) not be a past recipient of the award.


NOMINATION PROCESS:
The nominator is responsible for completing the nomination form and submitting the award nomination packet. The deadline for nomination materials is January 31 annually. The nomination packet is to include the following items:

Phi Lambda Sigma Outstanding Chapter Advisor Award


DESCRIPTION:

The Phi Lambda Sigma Outstanding Chapter Advisor Award was established in 2006 to recognize an advisor of a Phi Lambda Sigma chapter who has promoted student leadership, actively contributed to the achievements of their local chapter, and served as a leader and mentor through their service to the profession and/or the community.


CRITERIA:
The nominee must: 1) be a member of Phi Lambda Sigma; 2) have been an active chapter advisor for at least one year; 3) have actively promoted student leadership; 4) have made significant contributions to achievements of the chapter; 4) have served in leadership roles in the profession and/or the community; and 5) not be a past recipient of the award.


NOMINATION PROCESS:
The nominator is responsible for completing the nomination form and submitting the award nomination packet. The deadline for nomination materials is January 31 annually. The nomination packet is to include the following items:

Phi Lambda Sigma Chapter Alumni of the Year Award

Applicants are sought annually for the Phi Lambda Sigma Chapter of the Year Award. This award is for an alumnus of Phi Lambda Sigma who has exemplified leadership qualities in support of the pharmacy profession.

The individual chosen by the chapter for this award will be listed in the annual awards booklet at the awards event held in conjunction with the APhA annual meeting. In addition, the chapter will receive a certificate to present to the awardee. Finally, chapter awardees will also be considered for the Phi Lambda Sigma National Alumni of the Year Award if the chapter submits the nomination form for consideration. Please note that local alumni winners do not need to be alumni of the local chapter of Phi Lambda Sigma.

The individual chosen must have a proven track record of servant leadership in any one or more of the following areas; education, association work, community service, professional practice or mentoring. The individual must be a member of Phi Lambda Sigma.

Applicants are sought annually for the Phi Lambda Sigma Alumni of the Year Award. This award is for an alumnus of Phi Lambda Sigma who has exemplified leadership qualities in support of the pharmacy profession.

The individual nominated via this application must be an individual selected by a Phi Lambda Chapter as the Chapter’s Alumni of the Year awardee. Each individual chosen as a local awardee will be listed in the annual awards booklet at the awards event held in conjunction with the APhA annual meeting. The local winners of the award will also be considered for the Phi Lambda Sigma National Alumni of the Year Award. Please note that local alumni winners do not need to be alumni of the local chapter of Phi Lambda Sigma.

The individual chosen must have a proven track record of servant leadership in any one or more of the following areas; education, association work, community service, professional practice or mentoring. The individual must be a member of Phi Lambda Sigma.

The recipient selected will receive an engraved plaque which is presented during the Phi Lambda Sigma awards event in conjunction with the APhA annual meeting. Additionally their name will appear on the perpetual plaque maintained at the Phi Lambda Sigma national office.
