Appointment of Delegate and Alternate Delegate to the Phi Lambda Sigma Pharmacy Leadership Society for the 39th House of Delegates in Orlando, Florida on March 29, 2014. This form is for use in appointing the Chapter Delegate and Alternate Delegate for the Annual House of Delegates Meeting. Delegate appointment will also be available on site however pre-appointment is preferred.
